Sammanfattning
Coronary heart disease (CHD) is the leading cause to death in Sweden and in most European countries. Incidence and mortality of MI declined about 20% in Sweden and prevalence increase. Despite European guidelines patients with CHD show unfavourable lifestyle, medical parameters and beliefs not seeming to vouch for healthy choices. The gap between hospital care and primary health care (PHC) regarding follow-up of self-care is problematic. Nurses in PHC lack structured ways to follow- up patients after referral. Patients need pedagogical guidance. Patient-centred learning that takes the learning patients’ needs into account may stimulate healthy living. Nurse-led secondary preventive efforts in PHC have significantly improved recovery after CHD in Great Britain
During 2009 four PCH will be included in our study. One PHC is already involved. Vrinnevi hospital heart clinic want to collaborate with the PCH centres. About 160 patients will be enrolled during 2010-11 of which half will be randomised to controls and half to the PCE. Tutorial groups involving 6-9 patients meet 13 times during a year reflect lifestyle goals. Controls receive standard treatment which not involve PCE. Nurses work as tutors with a problem-solving process in PCE groups. The overall aim is to evaluate if patient-centred education affects self-care in relation to lifestyle goals included in the rehabilitation plan. Medical parameters, patient empowerment and visits at the general practitioner will be measured. Patients’ beliefs regarding lifestyle changes will be collected via reflective diary (during the educational process) and focus group interviews after 1 year and analysed qualitatively.
Results from this study are relevant for other PHC and educational programmes on all levels given by the Faculty of Health Sciences.
